My 6yo son’s breath is always horrid! I’ve asked his dentist and they’ve done x-rays and that’s not the issue. He mouth breathes at nights but can’t figure out how to get it to stop as dentists say ask the Dr, Dr says ask the dentist, ENT says possibly tonsil surgery but that may not solve bad breath. She said it’s to help behavioral issues/daytime sleepiness which aren’t an issue for him. No one will give us answers. Healthcare is so bad in Canada, we have to find the answers first then push the drs to do the testing so they can tell us the answer we said.
Maybe try mouth taping at night
My grandson had horrible breath. Got tonsils removed. No more bad breath.
Could it be tonsil stones?
It could be tonsils. It could be his gut health as well. Depending on what he’s been eating could be affecting his breath

Don’t tape a child mouth at night. How often is oral care being performed?
Look up the connection between good gut bacteria and bad breath. Also have him checked for diabetes. Another question is he constipated, that does affect breath.
I would insist on some bloodwork… it could be a wide range of things… diabetes, thyroid, tonsils, hormonal imbalance, tonsils , what he eats(allergies to something), allergy, intestinal, or gastrointestinal… sometimes hormones are off that leads to bad breath /odor.
Tonsillectomy usually works.
Look for tonsil stones. My 6 year old had to have her tonsils removed because of reoccurring stones… her breath was awful
Try pineapple juice after meals . That’s what a doctor told me when my daughter was young years ago
Get the tonsils out. Mouth breathing long term is a serious problem. It may not be causing issues now but it will. Also tonsils can hold tonsil stones which make the breath smell like rotten eggs. Do the tonsil surgery. It will help breathe and over all long term health. Clearly the doctor didn’t explain how mouth breathing negatively impacts facial structure development in children etc.
Try probiotics for kids first. He might just have bad bacteria in his gut and it’s comes out as stinky breath. It could be tonsil stones. Has the doctor ran blood tests to make sure everything is okay on that end if not then get your mama bear ready to fight for your son to get answers.
Bad gut bacteria. Feed them fermented vegetables
Could be allergies. Stomach issues. Tonsils or sinuses.
It could be tonsil pebbles. They end up in the back of the mouth and can make breath bad.
Try brushing his tongue or getting a tongue cleaner. Mouthwash if he can learn to not swallow it may also help. Flossing is important too. Especially the back molars. That can help get anything brushing missed.
It could also be diet or a digestive issue. More water, less processed foods.
Gut health can affect this. But one thing you said that stands out and I know is that mouth breathing causes dry mouth, which yes causes bad breath. They say that using a mouth wash that targets this helps a lot. Do you assist with their oral care?

When my niece was about 4 she had horrendous breath. Mom finally took her to the Dr. She had put chewed up bubble gum in her nose and it was spoiling. Dr pulled it…bad breath gone
Not drinking enough water.
I take it that there is tooth brushing… and mouth wash ??
It could be gut health. Do you give him probiotics daily? I’d definitely recommend if you don’t. A lot people don’t realize that bad breath is the result of bad gut health and not necessarily something to do with the mouth.
Charcoal tooth paste. Or powder.
